What happens when I contact Crossroads?

You can contact us directly or ask a your community Nurse or GP to make a referral. You can also request a Carers Assessment from Social Services.
We accept referrals from both social services and self funding individuals.
Our Care Coordinators will then arrange to visit to assess your individual needs. Each family can then be matched to a suitable carer support worker who, as far as possible, becomes the regular carer support worker for that family.
